NCQA is redesigning its PCMH Recognition program; the redesigned program will be launched on April 3, 2017. The last day to purchase the PCMH 2014 survey tool is March 31, 2017. Purchasing the PCMH 2014 survey tool will allow you to submit under the 2014 standards. Once your three-year recognition period ends you will then transition into the PCMH 2017 program. 

If you are planning to participate in the FCAAP Medical Home Transition Program, we encourage you to visit the program website to begin the process. As a reminder, these tools and resources are FREE to members of FCAAP!  

Renewing multisite practices should request the multisite interview with NCQA as soon as possible. To do this, log into your NCQA application and request the interview. The last day to submit corporate surveys will be May 31, 2017 and the last day to submit site specific surveys will be September 30, 2017. To learn more about the redesign process and understand your options, visit the NCQA redesign website. If you plan to submit under the PCMH 2014 program, please purchase your survey tool now. The survey tool will not be available after March 31, 2017.
